- Championship Roster – This writeable roster form can be printed and used as the Official Roster and Waiver of Liability. This 2 page form must be printed on front and back of the same sheet of paper. All information should be completed, including player signatures and initials. When obtaining signature from your local Indiana ASA District Commissioner, please have additional copies available so that he/she may retain a copy. For any questions regarding this roster form, please contact your local Indiana ASA District Commissioner.
- Indiana ASA Adult State Tournament Entry Form - Complete this form and have the form signed by your local Indiana ASA District Commissioner (whoever registered your team with Indiana ASA). Once complete, mail the form, along with your entry fee (money order or certified check), and Championship Roster to the Tournament Director so it is in his/her hands at least seven (7) days prior to the scheduled start of the tournament.
- Official Bid Form for Adult Championship Play - Complete this form and mail or email the document to the Indiana ASA State Commissioner (or his/her designee) by March 2, 2012 in order to be considered as a host site for 2013 Indiana ASA Adult Championship Play events.
- 2012 Player request for Reclassification - Complete this form and follow the procedure documented within to be considered for reclassification for the 2012 season. It is only necessary to mail, fax, or email page three (3).
